Repeat Tests at a New Hospital in China? The HR Recognition Rules
What the HR mark on a Chinese test report means, which results can be recognised across hospitals, the six situations that allow repeat testing, and how foreign reports are handled. Rules only.

Summary: China has applied the Measures for the Administration of Mutual Recognition of Examination and Test Results since 1 March 2022. A project that passes quality evaluation is marked "HR" with its recognition scope. If a result meets the conditions and the needs of care, the hospital may not repeat the test. Six situations allow re-testing, including a change in condition, rapid change in the result, major surgery, emergency care and forensic appraisal. A foreign report does not automatically carry the Chinese mark; the treating doctor decides whether to use it.
International patients often arrive in China with test reports from home or from another Chinese hospital. At a new hospital, the practical question is whether those results can be used or whether everything must be repeated. The answer depends on whether the report belongs to a quality-evaluated project, whether the result meets the needs of the current episode, and the treating doctor's assessment. Understanding the rules reduces unnecessary repeat testing and helps a patient ask why a repeat is needed.
What is mutual recognition of examination and test results?
It means hospitals accept quality-evaluated examination and test results that carry the mutual-recognition mark, so that the same test is not repeated unnecessarily.
Article 2 of the Measures defines an "examination result" as image or data information obtained by ultrasound, X-ray, magnetic resonance imaging, electrophysiology, nuclear medicine or similar methods. A "test result" is data obtained by biological, microbiological, immunological, chemical, haematological, biophysical, cytological or similar testing of material from the human body.
The same article states that examination and test results do not include a physician's diagnostic conclusion. What can be recognised is the image, data and objective result in the report, not the previous hospital's diagnosis. The diagnosis remains the responsibility of the treating doctor, who considers the history, physical findings and current condition.
What does the "HR" mark on a report mean?
HR is China's unified mark for mutual recognition of examination and test results. A label such as "National HR" or "Beijing-Tianjin-Hebei HR" shows that the project passed quality evaluation for that scope and is on the corresponding recognition list.
Article 13 provides that the mark is uniformly "HR". A project that takes part in quality evaluation by a quality-control organisation at any level and passes must be labelled with the corresponding "recognition scope plus mark", for example "National HR", "Beijing-Tianjin-Hebei HR" or "Beijing Xicheng District HR". A project that does not take part, or fails the evaluation, may not carry the mark.
Article 12 sets three recognition scopes:
- a project that passes national quality evaluation is recognised nationwide;
- a project that passes a local quality-control organisation's evaluation is recognised in that organisation's region;
- where different regions have an agreement, the project is recognised in the agreement area.
The scope label therefore matters as much as the letters HR. A report without the mark is not automatically unusable, but it normally falls outside automatic recognition; the treating doctor decides whether to accept it.
Which examination and test results can be recognised?
Projects should be reasonably stable, use a unified technical standard and be suitable for quality evaluation. The actual project list is compiled and published by local health authorities.
Article 11 sets the conditions for a project to be considered for recognition. Article 15 requires local health authorities to guide quality-control organisations in regularly reviewing the list and making it public. There is therefore no single unchanging national list; provincial and local lists may differ and may change as quality-evaluation results change.
For a patient, the order of checking is:
- Does the report carry the HR mark and a scope label?
- Is the project on the recognition list published for the hospital's area?
- Does the treating doctor consider the result adequate for the current episode?
Imaging data and laboratory reports fall within the definitions, but not every project is automatically recognised. The report label and the local published list are the practical reference points.
When may a doctor require a repeat test?
A repeat may be required when the condition has changed and the result no longer fits the clinical picture, when the result changes quickly, before major surgery or transfusion, in emergency care, for forensic or disability appraisal, or when a repeat is otherwise genuinely needed.
Article 19 lists six situations:
- the condition has changed, so the result is inconsistent with the clinical presentation or diagnosis and cannot support the care decision;
- the result changes rapidly during the course of the disease;
- the project is particularly important for diagnosis or treatment, such as before surgery or blood transfusion;
- the patient is in an emergency or rescue situation;
- the test is connected to judicial, disability or early-retirement appraisal;
- another situation in which a repeat is genuinely necessary.
Article 21 also requires the hospital and its staff to explain a decision not to recognise a result and to inform the patient of the purpose and necessity of the repeat. Before a repeat, a patient can ask what question the new test is meant to answer, why the earlier report is not adequate, and how the result will affect the next step.
Will a Chinese hospital accept a test report from abroad?
A report from a foreign hospital normally carries no Chinese HR mark and does not automatically fall under these recognition rules. The treating doctor decides whether to use it and may require additional tests.
The Measures apply to medical institutions within China, and the HR mark depends on quality evaluation by a Chinese quality-control organisation. The Measures do not provide for automatic recognition of foreign reports or for converting a foreign report into a Chinese recognition scope. A patient should therefore not assume that a hospital must accept the report, and should not read a decision not to recognise it as a judgment on the earlier doctor's work.
To make the discussion more efficient, bring:
- the original report or an official hospital copy;
- the imaging disc, original files or a readable link, not only a paper description;
- the report date, the method used and the reference range;
- a translation if available, while keeping the original;
- a short history and timeline of the relevant tests.
Whether to accept a foreign report and which additional tests are needed is a clinical decision for the treating doctor. This article gives no diagnostic advice and does not suggest that a patient should demand or refuse any particular test.
How are fees handled when a result is recognised?
If the result already meets the needs of care, the hospital charges the outpatient or emergency consultation fee and no separate examination fee. A hospital consultation fee may apply where several departments must confirm recognition; a genuine repeat is charged as the actual service.
Article 28 sets out three fee situations:
- where the result meets the needs of care, the outpatient or emergency consultation fee applies and no additional examination fee is charged;
- where the result meets the recognition conditions but several departments must take part to complete recognition, an in-hospital consultation fee may be added under the hospital's price policy;
- where the result meets the conditions but falls under Article 19 and a repeat is genuinely needed because the result cannot support the diagnosis, the actual medical service fee is charged.
Article 18 also requires doctors to order tests according to the condition and prohibits charging again by bundling the same item with other tests. The exact amount is a matter of the hospital's published prices and the actual settlement.
What can a patient do if a result is not recognised?
Ask the doctor to explain the purpose and necessity of the repeat, check the HR mark and the local published list, and use the hospital's published inquiry or complaint channel for billing questions.
Practical steps:
- Check whether the report carries the HR mark and a scope label.
- Ask which of the Article 19 situations applies.
- Check the recognition list published by the local health authority.
- For billing questions, contact the medical-affairs office, patient-service centre or the published price channel.
- Keep the report, payment receipt and correspondence for later checking.
The purpose of mutual recognition is to reduce repeat testing while protecting quality and safety, not to remove a doctor's ability to adjust testing to the condition. A patient has a right to information and to ask questions; the final testing plan remains a clinical decision.
